One of the most challenging issue with the opioid crisis is how to respond to a patient’s pain without causing harm. Ethically as nurses, we are required to treat pain “as whatever the patient is stating and is experiencing”, and we have a moral obligation to respond to the patient’s need and provide treatment. However, if we encounter the misuse of opioid medications by patients, it may cause a conflict in our ethical duty to provide care and at the same time to prevent harm. To be ethically and morally considerate it is necessary for nurses to understand the patient’s perception of pain, provide support and education, and offer pain medication as ordered. Additionally, offering patient’s non pharmacological therapies such as cognitive behavioral therapy, guided imagery, or one on one support may be effective strategies. Physicians also face this ethical issue as they must decide when it is okay to prescribe opioids. They must carefully prescribe pain killers to patients while at the same time preventing increased tolerance, dependency, and overdose.
Current Legislation
On July 22, 2016, President Obama signed the Comprehensive Addiction and Recovery Act (CARA) in to law because it addresses the full continuum of care from primary prevention to recovery support, including significant changes to expand access to addiction treatment services and overdose reversal medications (American Society of Addiction Medication [ASAM], 2018). The highlights of this law is that it grants money towards federally qualified health centers (FQHCs) and opioid treatment programs (OTPs), states to implement strategies for pharmacists to dispense naloxone pursuant to a standing order and to develop naloxone training materials for the public, to expand the availability of medication-assisted treatment in areas with high rates of heroin and opioid use, and requires Nurse practitioners and Physician Assistants to complete 24 hours of training to be eligible for a waiver to prescribe and must be supervised by or work in collaboration with a qualifying physician.
