Healthcare Program/Policy Evaluation Hospital Readmission Reduction Program (HRRP).
After scrutinizing the issues affecting patients and healthcare workers, I selected the particular program because patient readmission stood out as the most critical factor for patient outcome.
Description Healthcare providers should collaborate to provide quality patient care and patient outcome. Hospital readmission is a result of many factors. The increased pressure to treat patients within the shortest time possible could result in poor patient care and patient outcomes, leading to hospital readmissions. The Hospital Readmission Reduction Program aims at eliminating the rates and cases of patients developing complications within 30 days after hospital discharge.
How was the success of the program or policy measured? The program evaluation studied the physical, social, and financial impacts of hospital readmission on the patients and nursing providers. The success of the program was measured by analyzing the goals and objectives met by the program. Feedback from nurses, patients, and their caregivers was also obtained and analyzed to measure the success rate. The number of readmission cases was recorded compared to the previous years, where the program was not in use.
How many people were reached by the program or policy selected The program has reached all populations of patients. Most of the hospital readmissions cases happen to older patients. The program has effectively serviced all states in the country. In each state a minimum of 200 health facilities were targeted.
How much of an impact was realized with the program selected? There have been significant reductions in the cases of readmission for vulnerable populations after the program was effected. Improved patient care and outcome doubled as a result of the program. more patients recorded patient satisfaction due to reduced health care costs. Better communication between the health practitioners and patient or the caregiver was developed before and during the discharge period to avoid readmissions.
At what point in the program implementation was the policy/program evaluation conducted? The program evaluation was conducted after the program was created, analyzed and monitored, and implemented. The reason is that the program is required to meet specified medical standards and patient satisfaction.
What data was used to conduct the program or policy evaluation Several data were used to conduct the hospital readmission reduction program. Three medical conditions were included in the analysis of the hospital readmission reduction program, HF AMI and pneumonia. A total of 150 patients from the three states and above the ages of 65 were used to evaluate the program. Cases of patients with developed medical complications were identified, and the cases were analyzed individually.
What specific information on unintended consequences was identified HRRP was implemented in my facility of work to reduce the number of readmission cases. The efforts of preventing hospital readmission affected a number of patient care quality. The rate of patient mortality increased significantly after discharging from the hospital. Twenty patients under study died within 30 days of discharge. Patients under investigation were cared for in observation units as an alternative to inpatients units. This led to poor patient care resulting in their deaths.
What stakeholders were identified in the evaluation of the program? who would benefit the most from the results The program involved various stakeholders, namely; patients, nurses, pharmacists, physicians, care providers, hospital administration, and insurance companies. The most beneficiary stakeholder would be the patients. Patients reported that the program lowered their costs for treatments and improved the general quality of medical services.
Did the program meet the original intended and objectives The program met all intended objectives. The number of patient readmission reduced significantly. The number of medical errors caused by nurses in the hospitals was reduced by half. There were few cases of complications developed in discharged patients’ homes.
Would you recommend this program in your place of work I would recommend the HRR program to my health facility due to its enormous advantages. The hospital readmission reduction program has several benefits not only to the patients but also to all the other stakeholders.
Identify ways that you, as a nurse, would become involved in evaluating a program after one year of implementation Nurses need to assume leadership roles in program evaluation after its implementation. The first step I would conduct in the program evaluation is to engage the stakeholders, especially the patients. I would ask for their opinions on the new services they receive and how to better conduct healthcare provision. I could also contribute to the setting of new procedures and processes to be included in the project.

Program/policy evaluation answers basic questions about program/policy effectiveness. It involves collecting and analyzing information about program/policy activities, characteristics, and outcomes. This information can be used to ultimately improve program services or policy initiatives.
Nurses can play a very important role assessing program/policy evaluation for the same reasons that they can be so important to program/policy design. Nurses bring expertise and patient advocacy that can add significant insight and impact.
