Clinical Experience in an Interdisciplinary

ANSWER

Clinical Experience in an Interdisciplinary Team as a Registered Nurse

Multidisciplinary Group Members
I worked on an interdisciplinary team with the following professionals throughout this clinical experience:

As a registered nurse (RN), my duties included providing direct patient care, giving prescriptions, keeping an eye on vital signs, and making sure patients were comfortable.
A medical doctor (MD) is in charge of making the diagnosis and creating the treatment strategy.
A psychiatric mental health nurse practitioner (PMHNP) is a professional who focuses on managing a patient’s mental health, including prescribing medication and offering therapy.
Social Worker: Helped organize the patient’s release and attended to their environmental and social needs.
Pharmacist: Contributed to patient and team education and drug interactions.
Physical Therapist (optional, if applicable): Helped with physical rehabilitation and mobility.
The Patient Case and the Need for Cooperation
The patient, a 45-year-old man, was diagnosed with both generalized anxiety disorder (GAD) and diabetes mellitus. His elevated anxiety and uncontrolled blood glucose levels when he arrived at the emergency room made it difficult for him to follow his diabetes treatment plan.

Why cooperation was required:

Complex Needs: In order to treat the patient’s physical and mental health conditions concurrently, integrated care was necessary.
Treatment Adherence: He needed the PMHNP’s assistance to stabilize his mental health because anxiety was interfering with his capacity to control his diabetes.
Safe Discharge Planning: The social worker’s assistance was essential in guaranteeing that the patient had access to resources, including reasonably priced medicine and transportation to follow-up visits.
The Value of Teamwork in a Holistic Approach to Patient Care: A patient’s well-being is handled in all its facets through teamwork, which integrates social, psychological, and medical care.
Better Results: Coordinated efforts improve patient safety and health outcomes by closing care gaps.
Effective Use of Expertise: Well-rounded care results from the distinct talents and viewpoints that each expert brings to the table.
Patient-centered care places the patient’s needs and preferences at the center of the treatment plan through shared planning and decision-making.
Lessons Learned from the Encounter Value of Communication: To prevent poor management or effort duplication, team members must communicate clearly and consistently.
Respect for Roles: Being able to comprehend and value the knowledge of other experts improved our capacity for productive teamwork.
Patient Empowerment: Through collaborative care, the patient was able to take charge of his health management and feel supported.
Interdisciplinary Synergy: I became more aware of the value of collaboration in accomplishing patient objectives after witnessing how several disciplines operate in tandem with one another.
Because it gave me the opportunity to learn from my colleagues and expand my perspective on patient-centered nursing practice, this experience reaffirmed the importance of interdisciplinary teamwork for both professional development and patient care.
